• Have read the College’s prospectus and current conditions of enrolment, and:
• Acknowledge that if a place is offered and accepted, enrolment is bound by the Terms and Conditions of Enrolment

• Information provided by parent/carer(s) on this form is used to consider the application for enrolment and if a place is offered, to enrol the student at the College.
• The school will not disclose this information to anyone else without the approval of the parent/carer(s) unless it is necessary to obtain specialist advice in relation to a student’s needs on enrolment.
